Viafet tracks visits to our Websites

concerning our patients is used or disclosed through our LIS Portal.
Users - The LIS Portal is only for the use of physicians and their authorized
representatives as stated in the Terms and Conditions of Use for the LIS
Protected Health Information - The LIS Portal is used for the storage and
transmission of Protected Health Information between Viafet and physicians and
their authorized representatives. Protected Health Information is used in
accordance with the Health Information Portability and Accountability Act
(HIPAA) and applicable federal and state laws governing patient privacy.
Protected Health Information available on the LIS Portal may only be used or
disclosed for treatment and other authorized purposes as stated in the Notice
of Privacy Practices.
